Payment information

We accept patients that have an Enhanced Primary Care (EPC) plan from their GP. We require full payment upfront on a debit card, then the EPC rebate amount will be credited to the same account.

At Aligned Form we require payment at the time of your appointment. If you have any questions, please contact the clinic prior to your appointment.

HICAPS facilities are located on site and if you have private health you may be able to claim on the spot. For information regarding the amount of rebate you will receive please contact your own private health care provider.
